Please fill out the academic eligibility form.

1. Fill out the top section (Student & Activity Information) with the school your child attends AND the middle section with your child’s name and Williamson County.
2. Email filled out form to amy.hays@ag.tamu.edu or bring to the Extension office.
3. County Extension agent will sign the form and will then email the schools with the signed document.

Please send this form to CEO no more than two weeks prior to the 4-H event or as soon as possible.
