How to fill out the above Academic Eligibility Form:

1. Fill out the top section (Student & Activity Information) with the school your child attends.
2. Fill out the middle section with your child’s name and Williamson County.
3. Email filled out form to amy.hays@ag.tamu.edu or bring to the Extension office.
4. County Extension agent will sign the form and will then email the schools with the signed document.

Please send this form to CEO at least one week prior to the event, or as soon as possible.
